  • Online Application for Admission (Web form used to apply for Graduate Admission online)
    (The $25 admission application fee is waived if the online application for admission is submitted.)
  • Information Request Form (Web Form used to request that a graduate school information packet be mailed to you)
  • Attend an Information Session (RSVP web form to attend on a date and location most convenient for you)
  • Statement of Purpose Form - Online (Web form that is part of the Graduate Application and is submitted directly to the Graduate Office)
  • Download Reference Form**(Printable form to be given to respondents who will be recommending you. You will need to print at least two copies.)
  • Download Transcript Request **(Printable form to be sent to your undergraduate schools. You will need to print one copy for each school attended.)
  • Application for Graduate Re-Admission **(This application is for previously enrolled students in good standing who wish to re-enter DBU after an absence of more than 24 months and less than five calendar years. Scan the completed form, along with a current professional résumé to graduate@dbu.edu or fax to 214-333-5579. The $25.00 application fee will be waived with this application.)
  • Application for Graduate Special Admission **(This application is for students who hold the minimum of a Bachelor's degree or a Master's degree from a regionally accredited institution and are seeking special admission to take graduate courses at Dallas Baptist University for transfer to another university, or for the purpose of certification, licensure, or other professional objectives. Except in special circumstances, students will be limited to no more than 12 credit hours. All others must apply for regular admission.)
  • Certificate of Church Membership - Recommendation of church staff member or official for ministry degree applicants.
  • Certificate of Church Membership - Recommendation of church staff member for M.A. in Counseling applicants.
  • Online Application Fee Payment - Pay your $25 non-refundable application fee using a credit card.
    (The $25 admission application fee is waived if the online application for admission is submitted.)
